OSHA Recordkeeping: Form 300 (Log of Work-Related Injuries and Illnesses); Form 300A (Summary of Work-Related Injuries and Illnesses); Form 301 (Injuries and Illnesses Incident Report)

Diesel Permit Application

Citation and Notification of Penalty

This is what a citation and penalty notification from the Division looks like, and it includes important instructions, such as posting and the right to appeal.


Notice of Verification of Abatement of Serious Violations

The notice used when the Division has verified that a serious violation, Special Order or Order to Take Special Action has been abated.


Employer's Report of Occupational Injury or Illness (Form 5020)

Employers must report, within 5 days of knowledge, every lost-time injury or illness or those requiring treatment beyond first aid, to the State of California. (This is in addition to the immediate reporting required by Title 8 §342(a).)


Doctor's First Report of Occupational Injury or Illness (Form 5021)

Physicians who examine a patient with an occupational injury or illness must send 2 copies of this form within 5 days to the employer’s workers’ compensation carrier or the insured employer.


Document Request Sheet

The form used by the Division to request specific documents (such as safety programs, permits, records) to review after an inspection.


Employer's Signed Statement of Abatement of Regulatory and/or General Violations

Employers cited for violations must respond to the Division of Occupational Safety and Health with a signed statement that they have abated the violation(s) (unless an appeal is filed and the violation is on hold).


Chain of Custody

The form on which the Division keeps track of evidence it acquires/removes during an investigation, to establish that it has had constant control over the evidence and it has not been tampered with.


Variance Application Form

When an employer cannot comply with a safety order or believes it can provide equivalent safety in another way, it may seek a variance from the Cal/OSH Standards Board.


Activity Notification Form for Holders of Annual Permits (Scaffolding/Falsework/ Trenches/Excavations)

The Division must be notified by annual permit holders before work is started on buildings, structures, scaffolding, falsework, demolition, trenches and excavations. This is the notification form.


Special Order and Declaration of Service

Used when the Division finds a condition that is not necessarily citeable, but needs correcting.


Pesticide Illness Report

Physicians who examine patients with known or suspected pesticide-related illnesses must report to local health authorities. (Job-related exposures also require submiting a Doctor’s First Report.)

Notice of No Violation After Inspection

Notification to the employer that the Division found no violations on a specific inspection date.


Notice of No Accident-Related Violation After Investigation

Notification to the employer that the Division found no violations related to the accident/illness investigated.


Request for New, or Change in Existing, Safety Order

The form on which a request for a new, or a change in an existing, safety order is made.


Lead Work Pre-Job Notification

The Division must be notified by any employer of any construction work involving specified concentrations of lead before the project begins. This is the notification form.


Inspection Report

The form on which the Division records the initial facts regarding an inspection, such as employer information, reason for inspection, and dates of inspection and related conferences.


Appeal Filing Form

An appeal of cited violations and/or penalties, (also failure to abate notices, special orders or special action notices) must be filed within 15 working days of receipt of the citation (or other notification). An appeal must be filed for each contested violation, must be filed directly with the Cal/OSH Appeals Board in Sacramento, and must be accompanied by a copy of the citation.


Inspection Procedures

The checklist on which the Division records the progress of an inspection from initial contact through various conferences, including review of the basic requirements for IIPPs or other required written programs, postings in the workplace, and factors for penalty adjustment.


Employer's Signed Statement of Abatement of Serious Violations

Employers who are cited for violations must respond to the Division of Occupational Safety and Health with a signed statement that they have abated the violation(s), whether regulatory, general or serious, within 10 working days after the specified abatement period set by the Division (unless an appeal is filed and the violation is on hold).


Order to Take Special Action and Declaration of Service

Used to remedy an unsafe workplace condition covered by a safety order when enforcement is at the Division’s discretion, or to order an employer to pay for required safety devices.


Information Memorandum and Declaration of Service

Used to point out conditions which may be potentially hazardous to employees in the future.


Notice (in lieu of citation) and Declaration of Service

Used when the Division finds something that needs fixing but has no direct effect on employee safety or health.


Citation Declaration of Service

The form that verifies proper service of citations on the employer.


Notification of Failure to Abate

Used to follow up on unabated violations after a reinspection and to assess additional penalties.


Field Documentation Worksheet

Used by the Division to document the conditions found at an inspection site, including general physical conditions at the site or building, equipment and materials in use, and the type of business conducted at the site.


Permit

The permit to be posted by the employer specifies what type of permit it is and the conditions.


Investigation Summary

The OSHA form used also by Cal/OSHA that summarizes the accident/injury and related factors found during an investigation.


Order to Preserve

The tag placed at the scene of an accident or on the involved equipment not to be entered or disturbed, or to be preserved.


Notice of Accident-Related Violation After Investigation

The notification from the Division that it found a serious, repeat or willful violation related to an industrial accident or illness.


Narrative Summary

The facts of the accident being investigated, in narrative form, including date, time and location, information on the injured/killed, and the circumstances at the time.


Field Sanitation Compliance Form

 

If a farm employer has been cited for a failure to provide sanitary facilities, it must submit a completed form for five years, informing the Division of the number of workers employed and the type and number of required facilities it is providing.


Safety Representative / Gas Tester Application

Application for testing for either safety representative or gas tester certification.


Blaster’s License Application

Application to take the test for a blaster’s license.


Permit to Use Diesel Engines Underground

The permit issued for underground use of diesel equipment.


Notification of Tower Crane Activity Change

When there is an activity change for mobile or fixed tower cranes, such as climbing or dismantling them, the Division must be notified at least 24 hours in advance.


Notice of Tower Crane Deficiencies

Notice of deficiencies found during a tower crane inspection.


Tower Crane Operating Permit

The permit issued covering the operation of a tower crane.


Fixed Tower Crane Erection Permit

 The permit issued covering the erection of a fixed tower crane at a job site and its dismantling.


Tower Crane Operation Permit Application

 The application for a permit to operate a tower crane.


Fixed Tower Crane Erection Permit Application

The application for a permit to erect a fixed tower crane in excess of 36 feet.


Permit Conference Summary

Used by the Division following the permit conference covering projected activities and their hazards and what is to be done to minimize risk to employees.


Temporary Permit

The preliminary permit to allow an owner/builder to conduct a plan check preparatory to obtaining a building permit.


Declaration of Service - Order for Access

Request from Division for access to employee exposure and medical records.


Access to Employee Exposure and Medical Records

Verification of service of a request from Division for access to employee exposure and medical records.


Accident Report

The form on which the Division records the report of an accident involving  a fatality, serious injury or illness, pesticide-related poisoning, serious exposure or catastrophe.


Order to Preserve and Declaration of Service

The form used by the Division to order preservation of an accident site or physical materials related to an accident investigation.


Warrant Request Form

If an employer refuses to allow the Division entry to make an inspection, or withdraws permission during an inspection, the Division may request a warrant from the Superior Court to gain entry to a worksite.


Proposed Penalty Worksheet

The worksheet used by Division inspectors to document the calculation of proposed penalties for cited violations (or for failure-to-abate situations).


Order Prohibiting Use and Declaration of Service

Order from Division to Employer prohibiting use of an area or a piece of equipment in the workplace due to imminent hazard.


Complaint

 The form on which the Division records complaints received via telephone or in writing; it is then reviewed to determine whether an inspection is necessary (imminent hazard and formal complaints) or a letter to the employer will suffice (nonformal serious and non-serious complaints).
